What is Transitional Care

A short-term rehab program can be particularly helpful when it comes to transitioning from the hospital. Transitional care aims to help you make your best possible recovery and regain function as quickly as possible. Short-term rehab programs like ours generally provide physical therapy, occupational therapy, and speech therapy, as well as around-the-clock care.

What to Expect

While here, you’ll spend up to three hours a day with therapists. Most patients stay for up to 10 days, but that can vary depending on a person’s needs, goals, and current state when they enter transitional care. In transitional care, it’s a patient’s responsibility to make sure they are working toward becoming healthy again. Our team of specialists support the patient’s journey by developing a treatment plan that outlines achievable goals.
