The award-winning Rehab Select at Shelby Ridge provides physician-led rehabilitation, skilled nursing, and long-term care for patients throughout Shelby, Chilton, Bibb, and southern Jefferson counties, and communities south of Birmingham. Our dedicated, completely in-house clinical team delivers a range of services from stroke rehabilitation to wound care for patients ages 20 and up, supported by three dedicated therapy gyms — physical, occupational, and speech — built around each patient's recovery. The facility supports an active lifestyle for its residents and is involved in workforce development and community outreach across Shelby County.

Rehab Select at Shelby Ridge creates an individualized treatment plan for every patient.
Therapy takes place across three gyms, for occupational, physical, and speech therapy, using equipment that pinpoints each patient's weaknesses for a precise plan of attack, plus air-pressure resistance equipment that builds strength without the intimidation of traditional weights. Patients have access to up to three hours of therapy each day, and recover in large private rooms and outdoor courtyards, easing the emotional weight of a stay.
An on-staff physiatrist and orthopedic physicians round weekly, alongside weekly wound care physicians rounds and rounding from a psychiatrist and psychologist — with leadership staying visible on the floor, not managing from behind a desk.
Conveniently located near Shelby Baptist Medical Center, we help patients move from hospital to rehabilitation — and home — safely and confidently. Our 98.5% occupancy rate reflects that trust, with hospitals and families consistently choosing Shelby Ridge for care.
Physicians guide treatment planning and care coordination throughout each patient's stay, with wound care rounds led by a clinician — physician or nurse practitioner — on a weekly basis.

Shelby Ridge supports the next generation of caregivers through the Nursing Academy of Alabaster, a free program offering a pathway to CNA certification, along with therapy and dietitian internships and nursing apprenticeships in partnership with schools like Samford University and Central Alabama, Jefferson State, and Lawson State community colleges. The facility also participates in Project SEARCH with The Arc of Shelby County, providing internship rotations that help young adults with disabilities build workplace skills and pursue employment.
Rehab Select at Shelby Ridge accepts patients ages 20 and older, including those with tracheostomies — up to four patients at a time — and chest tubes. Our clinical team is equipped to manage complex medical needs as part of a comprehensive rehabilitation or long-term care plan.
